Nurses

Nurses You Can Trust
Skilled medical care is virtually an extension of the medical treatment and care received in a hospital or other medical facility but is provided in the comfort of the home environment. In-home skilled medical care services have been specifically prescribed by your physician.

We specialize in care due to aging, illness, recovery or rehabilitation, by providing an individualized service that you can trust in the comfort of your own home. Our nurses are knowledgeable have a wide range of expertise to accommodate various needs. Our goal is to help patients to function independently at home, to increase their comfort and to improve their overall health. Patients are eligible if they are homebound and under the care of a physician who certifies that they need one of more of the following:
  • Intermittent Skilled Nursing Care
  • Physical Therapy Services
  • Speech/Language/Occupational/Therapy
  • Pathology Service

We deliver services that enable elderly, chronically ill, disabled, and recuperating patients to receive the care they need in the comfort and privacy of their home. All of our services are provided with one goal in mind: to promote the highest degree of recovery, comfort, and independence.

Our care representatives are available 24-hours a day, seven days a week, so assistance is always at your fingertips.

Therapists

We aim to improve the physical, emotional and mental wellness, and quality of life of our patients by providing quality in-home care and services tailored to the individual needs of each patient. Everyone’s story is different. Our doctors take time to listen to yours. Your personal care plan is completely customized to helping you stay healthy

Faith Connections Physicians provides house-call services to home-bound patients. We service patients with active Medicare Part B, PPO plans, private pay (credit card) and a few other insurance carriers. Please call to see if we are enrolled in your plan.

Our providers include Medical Doctors, Nurse Practitioners, Podiatrists, and Wound Care Specialists.

Telehealth Visits Available 
Faith Connections patients are now able to have a remote visit with their Doctor via video using your cell phone, tablet or lap top. Ask for details when you call (708) 832-9423.


Types of Therapy

When you or someone you care about is in need of rehabilitation, our therapists will provide customized adult programs right in your home. Our team is trained to help patients with a variety of neurological, orthopedic, post-surgical and developmental diagnoses. As your needs evolve, so do our therapy services, helping patients reach their full potential.

Faith Connection’s therapy services include, but are not limited to, the following:

Physical Therapy (PT)
Physical Therapy is a rehabilitative service for the preservation, enhancement, or restoration of movement and physical function that has been threatened or impaired by disease, injury or disability. Physical therapy utilizes physical activity, exercise, physical modalities (massage, electrotherapy) assistive devices, and patient education and training to enable optimal performance and enhance health, well-being and quality of life.

Our physical therapists can evaluate, diagnose and customize a plan of treatments to help patients increase their mobility, strength, balance and coordination. Our therapists’ collaboration with multiple resources will maximize the patients’ capacity to do what is important to them in their daily activities.

Occupational Therapy (OT)
Occupational Therapy is a rehabilitative service that helps promote health and prevent injury, illness or disability with a focus on the tasks of daily living. Occupational therapy targets daily skills with modification or adaptation of the activity using sensory processing. As well as integration, strength, balance, coordination and visual perceptual enhancements.

Speech-Language Therapy (ST)
Speech-Language Therapy is a rehabilitative service that involves the treatment of physical and/or cognitive disorders that cause difficulty with communication and/or swallowing. Speech therapists evaluate and treat patients with a customized plan of interventions to develop and optimize an individual’s ability to communicate and/or safely maintain their nutritional intake in the comfort of their home. Regardless of the activity or need, our highly skilled staff ensures that every person has an empowering experience.

Speech-language pathologists help patients across the lifespan to develop their language and speech skills and increase their ability to communicate. This may also include comprehension skills, social pragmatic skills, and the use of alternative communication devices. The pathologists also educate patients and family members to promote safe and appropriate swallowing techniques and diets.

Medical Social Worker
Social workers commonly act as motivators in assisting people become more stable, both financially and emotionally. They often inform clients about eligible programs and help them with the application process.

Our social workers have direct access to a variety of information, such as housing programs, job counseling programs, job training services and financial assistance programs, and other Senior options. A few of our social workers may also help individuals access drug and alcohol rehabilitation programs and mental health counseling. Available services may vary.

Certified Nursing Assistants

Our Caregivers love their jobs, and work in this field because they are passionate about helping others. We care deeply about our clients, and treat them like family.

Our home health aides are extraordinarily compassionate and are able to form deep connections with those they care for. They are well rounded and have a profound understanding of nutrition, exercise and personal growth. Our home health aides perform a variety of solutions to the daily needs of senior adult patients.

Services include, but are not limited to the following:
  • Performing personal care (bathing, grooming, etc.)
  • Dressing
  • Preparing meals and feeding
  • Oral hygiene and skin care
  • Assisting with mobility and transfers
  • Incontinence care
  • Obtaining pulse, temperature, and respiration
